Video-assisted thoracic surgery (VATS) lobectomy has been increased with lower morbidity and pain, and same recurrence rate compare with standard thoracotomy lobectomy. Currently, more complicated surgical cases have been performed by VATS. Mediastinoscopic lymph node biopsy has been proven one of standard procedures for mediastinal stage for lung cancer. A 61-year-old man, received coronary artery bypass surgery(CABG) using left internal mammary artery, presented with 5cm sized lesion in the left lower lung on computed tomography. This lesion was highly suspicious as lung cancer. Mediastinoscopic lymph node biopsy and VATS left lower lobe lobectomy were performed safely. The pathologic result was pT2bN0M0 squamous cell carcinoma. He was followed up without recurrence for 4 years.
